Various of the floor being mopped after the operationSTORYLINEA team of American doctors and nurses has set up a field hospital in Bartella, northern Iraq, a short distance from the frontline fighting in Mosul. While the Iraqi army continues its push against Islamic State group militants in the city, the trauma centre, staffed by volunteers from the US, takes in the seriously wounded who are referred by hospitals and triage units closer to the fighting.  The clinic is paid for by the Samartan's Purse nongovernmental organisation, a Christian NGO.  The Samaritan's Purse medics have been recently dealing with a boy and a woman who were delivered to their gates by an Iraqi ambulance, bandaged and in pain.  Behind the blast walls is an emergency room and operating theatre, full of technology and expertise, to rival any city hospital in the US.  Their patients arrived with no names, no age, and with only sketchy details of what happened and how they came to be injured.Shelly Kelly, the hospital's Medical Services Director, is from Mustang, Oklahoma.  She heard they were both shot in a 'friendly fire' incident while making a bonfire outside their house in Mosul.One of the team believes the pair to be mother and son. While it doesn't change how they are treated, the team is energised by focusing on helping a family in peril. The clinic arrived in Iraq late in 2016, on December 25.  It was set up on the road between Mosul and the Kurdish city of Irbil. The centre is well placed to deal with wounded coming out of the eastern part of Mosul.  That half of town has now been cleared of IS, and the fighting is now in the western part of the city.   But with all the city's bridges across the Tigris river destroyed, the wounded from the west must now travel for hours, far to the south, before crossing the river and heading north again to the field hospital.      Several other clinics are now closer, and the majority of casualties are taken there first. Samaritan's Purse hospital usually receives those sent after initial treatment Ã¢Â€Â“ either because the others are full, or more often because the cases are difficult Ã¢Â€Â“ the injuries grave.  These medics have the equipment and expertise to help.  \"At home it's not near as critical as this,\" says Shelly Kelly, comparing her regular work in urgent care in the US, to the workload in Bartella. A mother of four, she has completed several assignments with the NGO, and is now on her second trip to Iraq. Going to a place where work is done behind blast walls, and danger is all around, took some effort to explain to her family.  In the end, they all support her, and understand her drive, she says.  \"My parents actually watch my kids, and wash my laundry when I'm not there, and make meals for my husband, so it's all good.\"The US staff come for rotation of between 4 to 8 weeks, most using their regular vacation time.\"It's really an easy choice,\" Shelly says, and adds the rewards are so much bigger than a vacation on a beach can ever bring. Paediatric surgeon Adrian Curnow from Boise, Idaho, is also in Iraq on his time off from his regular posting in the US. He started his first foreign mission in Bartella three days ago, and admits to being surprised by the severity of the injuries he's treating here. Curnow prepares to operate on the wounded boy.  He faces an issue common to all of his underage patients: where are the parents, and who will give consent for the operation?   In the confusion of war, it's easier said than done. \"We do our best to get family involved,\" Curnow says, and then like in the US, if two physicians decide it is an emergency: \"we do what the family would want us to do\".Unknown to the rest of the team, he manages to speak to the woman.  Before slipping into unconsciousness, she tells him she is not, in fact, the mother of the boy. So, he and a colleague consult, and decide to go ahead.  Both the boy, and the woman, are wheeled into adjacent operating rooms. Both patients have already been operated on by others.  The boy's abdomen needs to be cleaned, the surgery already complete.  But the female patient is in a much worse state. A bullet severed her spine, and she will never walk again. It also hit her kidney, and the surgeon decides to take it out. Before cutting, he checks the other side, and realises what a grave mistake that would have been. Her second kidney is missing Ã¢Â€Â“ either taken out overnight in the clinic which first treated her, or perhaps was removed years ago in a previous operation.   So, the team works to salvage what is left of the one remaining organ, and hopes for the best. The names, the details, the medical history  - an essential part of the workflow back home Ã¢Â€Â“ is often obscured in the fog of war, and the medics in the field learn to work around this. Both operations last more than two hours.  The staff are exhausted. Shelly Kelly grabs a moment to eat, and to reflect. Going abroad has changed her forever, she says, and so too all of her colleagues. With several such missions behind her, she knows coming home is not easy. \"I think it was anger, pushing my family away, and just trying to process through what I have seen here,\" she says about her last return to the US.
