SOUNDBITE: (English) Professor Jenny Donovan, co-investigator, ProctecT trial, University of  Bristol \"That's one of the problems, the men who were tested with PSA (prostate specific antigen) had no symptoms at all, the men who have a diagnosis of localised prostate cancer, the cancer is confined within the gland so they have no symptoms of it and then over time as the cancers grow, sometimes they do develop symptoms, but by the time they have symptoms of progressing disease, it's often too late to try and cure the cancer, you have to then manage it. So the whole point of this study really is to look at if you detect men with this PSA test early on, can you then treat it at that point and be successful.\"26. Low wide angle of Doug Collect sitting on park bench LEADIN: A report is released today on the world's largest trial into the best methods of treating men with prostate cancer.The report which has looked at more than 82-thousand men over a ten year period concludes monitoring is better for localised cancers than treatments which come with severe side effects.STORYLINE: Doug Collett is 73 years old and has been living with prostate cancer for several years.He wasn't actually diagnosed until after he agreed to take part in the trial run by the UK's National Institute for Health Research.The research followed more than 82 thousand men over ten years. Although new treatments are being investigated the options available to most men with prostate cancer is surgery, or radiotherapy. Both carry debilitating side effects which can affect the men's quality of life, so doctors are keen not to prescribe them if they can be avoided. A problem facing doctors is that many men don't experience symptoms until the cancer become too difficult to treat.The aim of the ProtecT trial has been to try to answer the question of when do the considerations about side effects become outweighed by the need for treatments. Collett had no idea he had cancer, but he was the right age group so he agreed to be tested.The result was a shock.  \"One of my samples showed a cancer and that was when it really hit home I suppose the consultant, just sat us down. Me and my wife were there and he said unfortunately you've got prostate cancer.\"Collett's first inclination was to demand the cancer was removed: \"I've got to get rid of it, I think that's what all men feel with anything to do with cancer. I think most people feel the same way, but having reviewed some of the literature that they gave me to read through the various scenarios from radical surgery, or radiotherapy, the side effects on some of that  is not very good.\"Trial doctors persuaded Collett to agree to a system of active surveillance, or monitoring.Doctors use monitoring because some men with prostate cancer never need treatment. Some prostate cancers develop very slowly, or flatline.Cancer Research UK describes post mortems have discovered prostate cancer in elderly men who have died from other causes.They hadn't had symptoms and didn't know they had it, rather like Collett.Blood tests and scans diagnose the cancer earlier, but it still not known whether the cancer will be aggressive and needs treating straight away, or whether it can be left.Despite first being in favour of treatment to start with, Collett is relieved he didn't insist on immediate treatment.He says: \"I think some of the side effects of the other two (therapies), the possible side effects, not everybody gets it, of course, which is impotency, incontinence doubly, bowel problems with the radiology (radiotherapy) stuff. We can't tell this with any certainty and that's why we need more research to identify that.\" In his report published in the New England Journal of Medicine Hamdy says what they need to know now is: \"What is the trade off between side effects that the treatment might produce and the longer benefits of curing the cancer and living longer and we don't again have the answer to that we need more time, because we've had so few deaths in the time, we've only had one percent of men who died.\" Co-author Professor Jenny Donovan says the impact of treatment side effects shouldn't be underestimated: \"Sexual function is affected in all the different groups because sexual function declines with age, so all the groups showed a decline in that, but again the surgical treatment and the radiotherapy treatment both have a severe impact on sexual dysfunction particularly in the first six months and then that continues. They recover a little, but the decline continues, whereas the active monitoring men have a steady decline in their sexual function.\"The problem though is that doctors and patients are still in the dark about whether in the long term, it will have been better to take the surgical route.Unfortunately, hindsight is of little use when a patient's cancer begins to rapidly develop.The study team now wants to investigate whether there should be routine screening for men aged between fifty and sixty-nine.Donovan says: \"That's one of the problems, the men who were tested with PSA (prostate specific antigen) had no symptoms at all, the men who have a diagnosis of localised prostate cancer, the cancer is confined within the gland so they have no symptoms of it and then over time as the cancers grow, sometimes they do develop symptoms, but by the time they have symptoms of progressing disease, it's often too late to try and cure the cancer, you have to then manage it.
