Cancer treatment can cause infertility and many young men must decide whether to bank their sperm for later use. Doctors say it's an added stress for boys - some yet to reach sexual maturity - who face months of arduous treatment. STORYLINE: At the age of 21 Joseph Mannix had everything to look forward to.After a happy childhood within a very close knit family, he had a job and was like other young men enjoying holidays and parties.That stopped last year when he became ill.Mannix was feeling constantly tired and breathless.He'd felt ill for weeks and after seeking medical advice he'd dismissed it as the flu.By this time Mannix was unable to stand up without passing out and his skin had turned yellow.He remembers: \"It got to the point where every time I stood up I fell down and it was basically I didn't have enough blood, or rather oxygen in my blood to keep me standing up. So every time I stood up I didn't have enough oxygen in my head so I fainted. So obviously with that going on my mum rushed me to the doctor's surgery.\"When Mannix was diagnosed with leukaemia his family were stunned.There was no obvious history of cancer and Mannix was young and had appeared healthy.His mother Janet Mannix says they were: \"Absolutely devastated. It hadn't entered my mind at all that it could be leukaemia, I thought pancreatitis, or I thought he'd injured himself in some way, or it just didn't cross my mind and it was a massive shock.\"  Doctors told Mannix that chemotherapy could leave him infertile. Though not in a relationship, he took the doctor's advice to bank his sperm.His mother is glad he did, but at the time it was the last thing they were both thinking of.Janet Mannix says: \"It was just like, sort of the bottom falling out of your world. It just wasn't anything you could even imagine, and then they did try to take him for an X-ray and he couldn't even sit upright in the wheelchair and passed out and I just kept thinking, he's going to die. And this is when he said to me, they got him back on the bed and the minute he lay flat he was okay again and I was like, terrible and I was crying and everything and he said to me you know, sort of, you got to chill out sort of thing.\"    It is important the sperm is taken as soon as possible before the chemotherapy starts.The drug works by killing quickly dividing cells. Sperm cells already divide quickly and that makes them them an easy target for the chemo and more likely to suffer damage.Once treatment starts sperm rapidly become less viable.When banked the sperm are stored in tanks like these.According to Allan Pacey, the Professor of Andrology at Sheffield University, cancer in young boys is a relatively rare event, but across Europe as a whole, doctors like him will still see tens of thousands of cases like Joseph Mannix and many are much younger. Because treatment has to begin as soon as possible there isn't much time for boys in this situation to consider issues like sexuality and fatherhood.Talking about having children and banking sperm is particularly hard if the patients are still children themselves.It's something Pacey has to face.He says: \"Now that's quite a remarkable and quite a difficult thing for someone of that age to think about, because not only have they got to process having received a life threatening diagnosis and understand the treatment that will be needed to cure them, but they've also got to process the fact that some of those treatment's may affect their fertility and may even make them sterile. So within a very short space of time a young man, or a young boy will have to understand the potential impacts for their fertility.\"In the UK, hospitals are allowed to keep the sperm for a maximum of 55 years and other countries have similar regulations.The aim is to make the sperm available for the men to use in future IVF or other reproductive techniques. Pacey explains: \"A cryoprotectant is added, that helps to protect the sperm when they're frozen and then the temperature is gently lowered to as cold as minus 196 degrees celsius and in tank's like this, these at that temperature, sperm can effectively remain in suspended animation for tens of years, maybe even hundreds of years and retain its viability.\" Pacey says the record for using frozen sperm successfully is held by an American who returned to the sperm bank twenty-eight years after banking it. He went on to father a child with his partner using a simple insemination. Those cases are rare though. Pacey says a recent German study looked at worldwide data and discovered only eight per cent of men who banked their sperm ever return to use it.He says this is good news because it shows over 90 per cent of men who have cancer treatment potentially go on to father children naturally.Joseph Mannix's doctors are hopeful he will also be able to father a child naturally.Mannix himself says male sexuality is still a taboo subject among young men, let alone teenagers:\"I'm a lot more mature than a thirteen, fourteen year old. A thirteen, fourteen year old has no level of sexual maturity. They're still watching sex education programmes at school. So it has profound consequences for the development of relationships.\"Any male above 13 is given opportunity to bank sperm, but even a 13 year old might not be sexually aware and may find the process too traumatic.Sperm before the age of puberty is not stored.Mannix is not an only child, he has a sister, but his parents are keen to be grandparents. However his illness and treatment has had many side effects on his health.His mother says: \"So each time there's another thing that's badly affected him (Joseph) you think, oh isn't leukaemia enough without having osteonecrosis and now if he couldn't have kids, but you've got to sort of go on what you've got, not what you'd like and you just have to make the best of what you've got.
