STORYLINE:It doesn't hand out cash, but this ATM machine has the potential to change lives here in South Africa.Access to a regular supply of antiretroviral drugs is vital for HIV patients.But here, even in the cities, the stigma of having the disease make many afraid of seeking medical help, let alone presenting themselves publicly at clinics to collect the necessary medication.This machine was one of the developments presented at a conference on AIDS in South Africa earlier this month (July).It dispenses medicines for chronic disease patients including the antiretroviral (ARV) drugs. Sub-Saharan Africa has the highest number of people living with HIV in the world, as well as the highest infection rate. It is estimated over six million people are infected with HIV in South Africa alone. That's why the programme to treat the disease here is the largest in the world.Despite the campaign, poverty and distance means many living with HIV still struggle to access clinics for prescribed drugs, or to receive health care. Hospitals and clinics also struggle to keep up with the demand for medication and people often have to wait in long queues for many hours to get their monthly dose of ARVs. The Managing Director of the Right ePharmacy initiative Fanie Hendriksz, explains: \"What you are seeing here is the pharmacy dispensing unit, it is a unit that dispenses chronic medicine directly to patients in a ATM format like this. It is made up of robotics and an automated system at the back, which allows easy access for medicine at convenient places for patients.\"The Pharmacy Dispensing Units (PDUs) work in a similar way to automated teller machines, but instead of providing an easy way to obtain cash, it provides instant access to vital medication. Here people can skip the clinic and pharmacy queues.What also differentiates PDUs from ATMs is that patients can speak to a pharmacist or a pharmacist's assistant responsible for dispensing medicine in real time on the screens of the machines.Medical practitioner Dr. Thapelo Maotoe believes the anonymity it provides is essential: \"What comes right at the top is the issue of stigma. Stigma we have been addressing for some time, we are addressing, but we are still having challenges in South Africa. So the PDU (the Pharmacy Dispensing Unit) by virtue of it being outside the practice, outside the clinic setting allows patients to go there in their own time, any time, any day, without really thinking about who is seeing them.\"Four of the Automated Pharmacy Dispensing Units (PDUs) or ATMs will be in operation in Alexandra township, outside of Johannesburg, from between October and November 2016.The ATMs at Thembalethu clinic in Johannesburg's Helen Joseph public hospital have also helped to cut the waiting time for patients.Maotoe says this is vital to free up more staff to treat a wider net of patients: \"So by decanting or reducing these patients we end up with facilities that can actually initiate more patients onto ART (antiretroviral therapy).Since the robotic system system was first introduced here in 2013 the average waiting time for patients has reduced dramatically from four hours to 45 minutes.According to Hendriksz: \"So for the in pharmacy solution that you see at Thembalethu, patients were used to waking up early to travel long distances to get here only to wait four to six hours for the medication. Now it is down to under 25 minutes and on a low volume day you could be out of here in five minutes. Raj Gudala has been a pharmacist at the Thembalethu clinic for three years, working with the automated in-pharmacy dispensing system.
