Wide of researchers in laboratory
Scientists claim they've transformed skin cells from two elderly men suffering heart disease into healthy heart cells in a laboratory.
The graft using an animal model is another step to perfecting stem cell technology which could one day help to tackle a disease which is on the increase in almost every region of the world.
It's a highly magnified spot under the microscope.
The bit that's moving is a cluster of heart cells and they're seen to be beating like a heart.
Researchers in this team took skin cells from a very sick patient with diseased and dysfunctional heart cells.
These were then reprogrammed to become induced pluripotent stem cells, which given the right cues and growth media can make cells for a number of organs.
In this laboratory they're creating heart cells, and more significantly they're healthy heart cells despite coming from a sick patient.
The most obvious benefit of this form of therapy is that there would be none of the tissue rejection normally associated with transplants.
Lead researcher Professor Lior Gepstein is a cardiologist as well as being an electro-physiologist.
He splits his time between Haifa's Rambam Medical Centre and the the Technion-Israel Institute of Technology.
Gepstein says: "The heart cells that were generated were young and healthy and resembled the heart cells of that specific patient at the day that he was born in contrast to the heart of the heart failure patient which was extremely dysfunctional and all the cells were very, very dysfunctional."
He's particularly encouraged by the fact that: "We also demonstrated that the cells could be engrafted into the in vivo heart in an animal model and that the transplanted heart cells could integrate with the existing tissue and beat in synchrony with existing tissue."
But just because the muscle beats like a heart doesn't mean that Gepstein and his team don't still face enormous obstacles.
Producing this small amount of muscle in a petri dish requires the production of a million, or so cells.
To even begin thinking of regenerative muscle for a human heart would require many billions.
It's also important for the team to ensure any tissue they produce is not predisposed to the growth of cancer tumours.
A major drawback for these researchers at the moment is that the heart cells they produce don't have a long shelf life.
There have been remarkable advances in heart surgery over the past two decades, but Gepstein is still convinced that ultimately doctors will need to turn to stem cell therapy.
He argues:
"Despite the significant improvements that were done in the last decade, or two decades in cardiology that involves medical treatment, surgery, coronary interventions, still the number of patients that have heart failure is continuing to grow, more patients survive heart attacks, more patients live to an older age and the number of patients suffering from heart failure is continuing to grow."
Gepstein says: "Bypass surgery does not regenerate a dead area in the heart. It can improve the vascularisation, improve the functions of suffering cells, but it cannot replace cells that died that were replaced by a scar."
Heart disease is the fourth biggest killer in the world according to World Health Organisation figures.
It caused 57 million deaths last year.
But research into stem cell therapy is not a quick, cheap option.
Getting these skin cells to become these stem cells and ultimately this muscle, has relied on decades of accumulated knowledge from research in laboratories like this around the world.
Gepstein says:
"We know that there are certain cues that guide the different creation of early cells to become heart cells to not become liver cells and so on, for example we know that the area that will become the stomach in the developing embryo secretes certain that affect neighbouring cells to become the heart. That's why people say the ways to a man's heart is through his stomach."
That may be a joke with more than a pinch of truth about it.
Gepstein is aware that turning what's happening here into regenerative medicine in a hospital will be a very long haul and he doesn't underestimate the difficulties.
He says: "We are only making cardiac cells and tissue and we find it extremely difficult. Imagine making an entire organ from different cells, already in the right place so it lets you appreciate the biological system and process that are involved in development that have been perfected
