Unlike other solid organs that die within minutes of stopping circulation, lung tissue and lung cells live on for hours after death."
Egan wants to ensure that doctors don't miss the opportunity to secure a donation in that window of time to allow people who die suddenly to donate their lungs.
The problem is that there is no process for harvesting the organs from donors who die suddenly outside of a hospital.
Up to 80% of lungs that are donated aren't usable.
Most come from donors who died in a hospital, where lungs are frequently damaged by ventilators and other complications during prolonged stays.
Egan wants to reach sudden death victims in their homes, or elsewhere in the community and he's testing the practice in parts of North Carolina.
"There are some logistical hurdles," says Egan.
He adds: "There are a lot of changes to the routine of handling individuals who die suddenly. On the other hand, a lot of individuals would like to be organ donors, and those who die suddenly generally aren't organ donors."
After someone collapses with cardiac arrest and emergency workers have exhausted attempts to resuscitate Egan wants teams to check whether the dead patient's driving license identifies them as as a registered donor.
If they are the local organ recovery agency would be immediately alerted to contact the next-of-kin for permission for to use the organ in the research.
If that happens within about an hour of death workers pump a little air into the lungs to preserve them while the body is transported to an operating room for organ recovery.
The next crucial step is to ensure the lungs are healthy through an examination in the laboratory.
This is the study's first pair of donated lungs.
Their lifetime after the donor's death is being increased by a process called ex vivo lung perfusion.
The machine infuses air and fluids through the lungs so they also can be inspected for disease and malfunction.
The black speckling on the outside of the lungs doesn't worry the team although they're a sign of city living, or perhaps smoking.
It's the inside that counts.
These lungs didn't pass the test because Egan found some early disease.
But they did show it was possible to get a donation in time to see if it was viable for transplantation.
"Right now in the United States, we do 1,800 lung transplants a year.
