Wide of airliner after take off (shot through perimeter fence)
Travellers going on long haul flights are being told they don't need to worry too much about Deep Vein Thrombosis.
DVT is commonly known as economy class syndrome, having previously been associated with passengers travelling long periods in cramped conditions.
But doctors say that's not the case.
Colin Deane is an expert at searching for Deep Vein Thrombosis.
It's a blood clot blocking a vein usually around the calf, or thigh muscle.
When all, or part of a clot breaks off and travels to the heart, or lungs, it becomes most serious and life threatening, this is called a pulmonary embolism.
On the ultrasound screen you can see how Deane is pressing down hard on the veins in one of his student's legs.
When the blood is flowing freely, it's easy to collapse the vein, so the black circles momentarily disappear.
A hard clot wouldn't do this.
This is a specialist thrombosis unit at King's College Hospital and a great deal of research on the subject goes on here.
Deep vein thrombosis, or DVT, became synonymous with flying, earning the moniker "economy class syndrome".
Airport terminals throb with passengers squeezing onto jet airliners going all over the world.
Concern was raised when passengers claimed to be suffering DVT as a result on being crowded into tight spaces.
In 2007 the World Health Organisation concluded a six year study claiming a "higher risk" for people travelling longer than four hours.
It put the risk at one in six thousand cases.
Some of the research found people taking multiple flights over a short period are also at a higher risk.
According to the WHO this is because the risk of venous thromboembolism doesn't disappear at the end of a flight, in fact the risk is "elevated for about four weeks".
But experts at the Thrombosis Centre at King's College Hospital urge holidaymakers not to be alarmed.
Consultant haematologist Roopen Arya insists "the risk is very small".
He says "you are a thousand times more likely to get DVT after being in hospital, for example, for surgery, or with an illness, so the actual risk of having a DVT after a long flight is only something like one in five thousand and the risk of having a life threatening blood clot, or venous thrombembolism associated with long haul travel is less than one in two million."
The student taking part in the demonstration is typically low risk.
He's not overweight, he hasn't undergone major surgery and he has not suffered previous clots.
Arya says those who's risk is elevated by these factors can take "additional preventive measures, for example, the use of elastic compression stockings which again should be of the right size and well fitted."
He says there is "a very select group of patients who are identified to be at very high risk of blood clots", particularly those who've recently had surgery.
Arya advises these patients should see their family doctor before travelling and consider "the use of an agent to thin the blood down before the flight and the customary treatment would be a single injection of heparin, which is a drug which prevents blood clotting, prior to the flight in each direction."
Heparin, an anticoagulant, is now widely used to thin the blood.
This scan shows the leg of a patient undergoing treatment.
Deane show how the veins don't compress, so the black holes on the screen stay intact, they don't collapse.
A scan like this raises the alarm straight away, it's a large clot in the knee.
Deane says "that's really quite a large clot for someone to have and poses a risk, so we would then refer this onto the haematologist for some therapy."
The group at the highest risk of DVT is pregnant women.
Britain's Royal College of Midwives says these women are advised not to use air travel in the first and last weeks of pregnancy.
Arya agrees that pregnancy is a risk factor in itself, pregnant women are more likely to suffer DVT and travel simply increases that risk.
But he says the risk itself is still low.
According to Arya "you need to be aware of the risks and ensure that you take precautions to prevent DVT and those precautions would be clearly, remain mobile, make sure you're well hydrated and certainly consider the use of compression stockings."
Plenty to drink doesn't mean sipping several of the onboard cocktails.
Alcohol dehydrates the body which increases risk factors for DVT.
Arya is also keen to debunk myths that have risen around DVT.
Your GP may prescribe a tablet of aspirin a day for all manner of ailments including high blood pressure.
But Arya warns not to expect that this will have the slightest impact on DVT.
He says "people are often advised to take a tablet of aspirin before long haul flights, but aspirin is really not very effective at preventing DVT and in fact can cause more irritation through the gastric irritation that a lot of patients might suffer."
Arya says everything is a balance and perhaps we shouldn't discount the effects of the relaxing holiday
