Mid of man smoking in street
Scientists believe it's possible to predict which smokers will find it the most difficult to give up cigarettes and which of us will succeed.
Studies worldwide are examining whether connections in the brain, and the way we behave, determine the best ways to help those resolving to crack the habit in 2013.
Lighting up can cause heart disease and damage your lungs, it can also cause cancer and gum disease, yet millions of us around the world still smoke, even those of us who want to give up. Now scientists are helping doctors to discover why.
Parts of the world with lenient tobacco legislation tend to have the heaviest smokers.
The most recent World Health Organisation (WHO) figures show that more than half of the male population in the Western Pacific region smoke, that includes all males from as young as fifteen years old.
The same WHO figures show smoking among women is highest in Europe at 22%.
In the UK, laws banning smoking inside all public buildings have forced addicts out onto the streets.
It's supposed to give them a greater incentive, especially in the misery of winter, to give up.
Scientists like clinical psychologist Professor Jane Powell believe it's possible to work out in advance which of us will be able to give up the habit and which of us are inclined to fall down at the toughest hurdle, the first week.
She's focussed her studies on brain injuries and addiction and become particularly interested in the brain mechanisms that appear after neurological injury and because they're similar to the ones that appear in addiction.
Powell says there are adaptations within pathways to the brain involved in motivated behaviour, just like an athlete in training.
Eating, drinking, sex, gambling , even country walks , the things that give pleasure in life are the same pathways that are adversely affected by periods of use of addictive substances
Powell and her team here at Goldsmiths University recruited 145 smokers and asked two thirds of them to give up.
They wanted to measure how people respond to motivation and see whether certain types of people showed greater motivation.
The team also worked on the assumption that smoking causes adaptations in particular pathways in the brain and this leads to changes in the way we are motivated to behave in all sorts of ways.
The active drug smokers are addicted to its nicotine, it has the effect of increasing the level of dopamine in the brain.
This is a neurotransmitter, or chemical messenger which, put simply, tells our brains we feel good.
If you cut the smoking you cut the dopamine level in your brain, this is when withdrawal symptoms are felt.
Powell and her team had a number of hurdles to overcome.
She says: "My interest was in testing out these hypotheses by seeing what happens to smokers in particular when they were deprived of nicotine for twelve hours and then to see whether any abnormalities in their cognition and affective emotional experience during those periods of abstinence predicted whether they would be able to quit more, or less easily."
Goldsmiths isn't the only University studying smoker's behaviours.
Here another British university, Exeter subjected smokers to intense exercise before carrying out a functional MRI (fMRI) scan of their brains.
During the scan there were shown pictures of cigarettes.
Those who'd taken part in the exercise regime were less likely than those outside the control group to suffer drop of dopamine.
Each of about 80 smokers had been subjected to detailed questioning about their likes, dislikes and what made them want to do certain things before the they quit smoking.
At Goldsmiths rather than looking at chemical messengers the team offered smokers an array of incentives including money to stay off the tobacco.
Powell says: "People who use substances, or engage in a variety of addictive behaviours tend to be quite impulsive, they're not very good at imposing the brakes, rational controls on the things they do. That's a key note of addiction really, people engage in drug taking even though they know it's a problem for them and the adverse consequences outweigh the benefits."
This reasoning supports restrictions on advertising the UK government has already introduced.
Supermarkets are banned from displaying cigarettes like this and in 2016 small corner shops like this will also be banned from putting them on show to the public.
The aim is to stop the impulsive act to smoke.
With the study Powell says: "The people who were most impulsive in the early stages of smoking abstinence were the people who found it most difficult to remain abstinent, so they were more likely than other people to have relapsed by a week so one possibility is identifying the people who are the most impulsive."
Based on this Powell and her team argue that it's possible to test for the most addictive personalities because they'll be the types least likely to think through their actions.
Powell believes that anti-smoking programmes could be more successful if they identified the most impulsive people before they give up, because abstaining for the first week is crucial to the long term success of the effort.
She suggests these people could be warned about their weaknesses in advance.
Another way to enhance their resolve is to ensure they're more alert.
Powell argues being alert helps us inhibit unwanted behaviour.
She says "If people are at risk because they're quite impulsive and by the way this is something that doesn't seem to recover with abstinence, then an important implication for those people may be to systematically try and do things particularly in that initial early risk phase to make them more alert."
She advises: "Do more exercise, go to bed reasonably early, don't burn the candle at both ends leaving you tired and therefore less able than you normally would be to inhibit those automatic behaviours."
Ultimately though, whether you're the sort who rushes in, or the type who likes to mull things over there is one thing the scientists can't help with and that's the determination and desire to quit in the first place.
Clients are reminded:
