Justice John Roberts. \u003cbr/\u003eDemonstrators for and against the law crowded the grounds outside the Supreme Court Building on Capitol Hill as Roberts, sitting at the centre of the nine black-robed justices inside, announced the decision to a packed courtroom.\u003cbr/\u003eProfessor Joel Teitelbaum, from George Washington University's School of Public Health and Health Services, described Thursday's ruling as almost as monumental as the first Congressional passage of the Affordable Care Act (ACA).\u003cbr/\u003e\"All of the things that were so profound in terms of the social policy changes around health and health care and public health remain today as a result of the decision,\" he said.\u003cbr/\u003eChief Justice Roberts' vote, along with those of the court's four liberal justices, preserved the largest expansion of the nation's social safety net in more than 45 years, including the hotly debated core requirement that nearly everyone have health insurance or pay a penalty. \u003cbr/\u003eThe aim is to extend coverage to more than 30 (m) million people, who now are uninsured.\u003cbr/\u003eThe 5-4 decision meant the huge overhaul, still taking effect, could proceed and pick up momentum over the next several years, with an impact on the way that countless Americans receive and pay for their personal medical care.\u003cbr/\u003eIn New York's Harlem district, patients at the Family Health Care Clinic reacted cautiously to the news of the Supreme Court ruling.\u003cbr/\u003eLeola Williams, who has worked at local hospitals all her life, welcomed the news while she waited for a check-up.\u003cbr/\u003eDoctor Nadya Hasham, a primary care physician, expressed concerns about costs but thought it was \"a positive move overall.\"  \u003cbr/\u003eAnd nursing home worker Mavis Salmon hoped that the news would help her mother who is having to pay directly herself for diabetes treatment.\u003cbr/\u003eIn Chicago, 35-year old David Zoltan is currently in a federally funded health insurance plan for people with pre-existing conditions.\u003cbr/\u003eDuring the recession, Zoltan lost both his job and health insurance.\u003cbr/\u003eWhile uninsured, his diabetes sent him to the emergency room three times when he ran out of insulin.\u003cbr/\u003eIn 2010, he was one of the first to sign up in Illinois for a new health insurance programme for people with pre-existing conditions.\u003cbr/\u003eDescribing the new programme as a lifeline, Zoltan said he was dreading the thought of the ACA being overturned by the Supreme Court.\u003cbr/\u003eIn Washington, DC law school graduate Samantha Ames also welcomed the Court's decision.\u003cbr/\u003eAmes was covered under the new ACA provision that allowed college students under the age of 26 to be covered by their parent's health care insurance.\u003cbr/\u003eAmes required surgery after tripping over her dog Harvey and was thankful she was \"able to get back on my parents' health insurance.\"    \u003cbr/\u003eSeniors in a nursing home in Pembroke Pines, Florida, had conflicting views over the result.\u003cbr/\u003eWhile 71-year-old Mary Watson thought the split decision reflected the state of the country, another resident, Irma Castro, disagreed concerned that her children won't be able afford to pay for health insurance.\u003cbr/\u003eThe health care overhaul focus will now quickly shift from Washington to state capitals. \u003cbr/\u003eOnly 14 states, plus Washington D.C., have adopted plans to set up the new health insurance markets called for under the law. \u003cbr/\u003eCalled exchanges, the new markets are supposed to be up and running on January 1, 2014. \u003cbr/\u003ePeople buying coverage individually, as well as small businesses, will be able to shop for private coverage from a range of competing insurers.\u003cbr/\u003eMost Republican-led states, including large ones such as Texas and Florida, have been counting on the law to be overturned and have failed to do the considerable work needed to set up exchanges. \u003cbr/\u003eThere's a real question about whether they can meet the deadline, and if they don't, Washington will step in and run their exchanges for them.\u003cbr/\u003eIn contrast to the states, health insurance companies, major employers, and big hospital systems are among the best prepared.
