of the accident.\u003cbr/\u003eOne member of the Columbia Accident Investigation Board said the report - due to be released on Tuesday - will dig deep into the so-called \"organizational culture\" of NASA and address problems that go beyond the management decisions linked to the accident.\u003cbr/\u003eThe report will detail how the shuttle was destroyed and analyse the management and engineering decisions that led to the accident.\u003cbr/\u003eIn one test conducted last month, a piece of foam was shot into the leading edged of a mock-up of the shuttle wing.\u003cbr/\u003eThat foam chunk put a hole almost 2 feet (60cm) in diameter in the mock wing, much more damage that was originally thought might have been sustained by Columbia's wing when a similar piece fell off during launch.\u003cbr/\u003eJohn Logsdon is director of the Space Policy Institute at George Washington University.\u003cbr/\u003eHe is also a member of the commission that investigated the accident.\u003cbr/\u003eIn preliminary releases, the board has already criticised some management decisions during Columbia's last flight.\u003cbr/\u003eThe commission has called for new rules requiring spy satellite pictures of the shuttle in orbit and for the development of a method for carrying out repairs in orbit of heat shield damage that might occur during launch.\u003cbr/\u003eSeven astronauts died on February 1 when Columbia disintegrated over Texas while returning from a 16-day orbital mission.\u003cbr/\u003eInvestigators determined that heat shield panels in the spacecraft's wing were probably broken during launch by the high-speed impact of foam insulation that broke off the external fuel tank.\u003cbr/\u003eIn hearings, the board learned that NASA managers accepted with few questions a flawed engineering assessment of the possible damage. \u003cbr/\u003eManagers also failed to order spy satellite photos of Columbia to look for damage, even though some low-level engineers recommended that.\u003cbr/\u003eAdministrators at NASA are warning employees that the report will be harsh, and say it should not be taken as a personal affront, but as a plan to get the
