We don't have enough money  from our own budget, so to continue to help people and not stop the medical process,  we take those patients who pay a lot of money faster than the others.  Much faster.\"\u003cbr/\u003eSUPER CAPTION:   Valery Shumakov, director of Moscow's Organ Transplant Institute\u003cbr/\u003eSasha is next in line to get a new heart, but he's been waiting for the last four years.\u003cbr/\u003eHe knows if he had the money, the transplant would have been a formality.\u003cbr/\u003eThere are more than 15-thousand people like Sasha throughout Russia, and not enough  hearts to go around.\u003cbr/\u003eSasha can't afford to buy a new heart -- all he can do is wait until it's his turn.\u003cbr/\u003eHis condition is so bad he might soon have to be attached to an artificial heart machine.\u003cbr/\u003eAlthough he has faith in the institute, he knows his chances are slim.\u003cbr/\u003e\"It could happen that I won't live long enough to get a heart transplant, because of all  the problems the country is having now with transplants. I know several people who  stayed here and died because they didn't get organs in time.  In this very same room.\"\u003cbr/\u003eSUPER CAPTION:   Sasha, heart transplant patient\u003cbr/\u003eBut at least Sasha lives in Moscow and doesn't have to travel far to get treatment.\u003cbr/\u003eSince this institute is the only place in Russia that performs heart transplants, heart  patients who live in remote Russian villages have to endure 18 hour train journeys to  come here for just a check-up.\u003cbr/\u003eTheir village doctors don't want to take responsibility for their health once the patients  have begun treatment in Moscow.\u003cbr/\u003eOnly a few of them can afford the constant trips to the capital, and they never get the  necessary help.\u003cbr/\u003eVery few people in Russia volunteer to donate their organs after they die.\u003cbr/\u003eRussia doesn't have a donor-card system or enough money to start educating people  on the benefits of organ donation.\u003cbr/\u003eTo make up for the organ shortage, the institute takes out organs from donors without  them ever knowing about it.\u003cbr/\u003e\"If we start the donor card system, or even worse, if we have to get permission from  relatives everytime, then the transplant situation will even get worse.  People aren't yet  educated about this.
