Our people went to nearby neighbourhoods and distributed syringes among addicts and encouraged them to come to Darvazeh Ghar's DIC centre. They gradually got more and more encouraged to come to that centre and receive methadone. It was the base of methadone and needle exchange program. Later on, this programme was expanded and with the open-mindedness of Iran's New Police Chief (General Ismaeel Ahmadi Moghaddam) the program speedily developed. Since I was an artist before and had art activities in the field of traditional music and Moaragh Kari (inlaid wood work) and Dr. Mohraz was aware of it, I came here on her request to train under treatment (HIV positive) patients. Pull focus of AIDS sign\u003cbr/\u003eIran may be a socially conservative country, but its surprisingly progressive drug programmes have helped to tackle the soaring number of AIDS cases.\u003cbr/\u003eNow doctors in the country are urging officials to take the same courageous and enlightened approach to deal with the spread of AIDS through unsafe sex, with up to 100,000 Iranians suffering from HIV.\u003cbr/\u003eThis man is one of Iran's growing numbers of AIDS patients.\u003cbr/\u003eThe government's own figures estimate that there are anything up to 100,000 thousand HIV carriers in Iran.\u003cbr/\u003ePositioned relatively close to Afghanistan and its poppy fields it's surprising that the number of drug addicts and consequently the growth of AIDS sufferers is a difficult battle to win. \u003cbr/\u003eDoctors at this AIDS Research Centre in Tehran are treating an increasing number of patients.\u003cbr/\u003eWhen AIDS arrived in Iran, the virus first hit the country's heroin users.\u003cbr/\u003eTo curb the outbreak and prevent it from spilling into the general population, leaders in the authoritarian regime adopted a decisively progressive strategy.\u003cbr/\u003eSo much so, that Iran is now being held up as an example of how to reduce the impact of addiction on health through diseases like AIDS. \u003cbr/\u003eFor years, Iran had a hardline drugs policy and it still executes people for certain drug trafficking crimes.\u003cbr/\u003eNow worldwide, experts are watching with interest the Islamic republic's burgeoning number of methadone clinics and needle exchange programs.\u003cbr/\u003eEspecially as these facilities are being squeezed in many western countries whose budgets have been cut by the world's financial crisis.\u003cbr/\u003eThis is one of the many drop in treatment centres rapidly spreading across the country. \u003cbr/\u003eTwo hundred publicly supported methadone clinics have been set up wean addicts off heroin and more than a thousand private clinics are offering the treatment.\u003cbr/\u003eCentres like these are run by Iran's Drugs Combating Committee.\u003cbr/\u003eCustom made machines automatically dispense condoms and fresh needles, these exchange programs are crucial if addicts like this man are to stay alive.\u003cbr/\u003eThe programme swung into action in 2005 after religious leaders issued fatwas declaring that drug users should not be prosecuted, provided they sought help. \u003cbr/\u003eThe courts supported the initiatives to protect the wider society and warned they shouldn't be blocked.\u003cbr/\u003eThat was vital in order to stop the police imprisoning drug addicts who attended treatment centres.\u003cbr/\u003eProfessor Minoo Mohraz heads the AIDS Research Centre.\u003cbr/\u003eShe says the turning point was when a drop-in centre was set up in Darvazeh Ghar, a poor neighbourhood in the south of Tehran.\u003cbr/\u003eMohraz says \"our people went to nearby neighbourhoods and distributed syringes among addicts and encouraged them to come to Darvazeh Ghar's DIC centre\". \u003cbr/\u003eAccording to the Professor \"they gradually got more and more encouraged to come to that centre and receive methadone. It was the basis of methadone and needle exchange program\". \u003cbr/\u003eBut women like this patient will continue to be at risk because she didn't know her husband was infected.\u003cbr/\u003eShe is HIV positive and keen to keep her identity hidden.\u003cbr/\u003eShe says she became ill through sexual relations with her husband, a drug addict who'd become infected while serving a jail term.\u003cbr/\u003eNow she suffers too, a clear example of how punishment without treatment hasn't worked.\u003cbr/\u003eDrug abuse is rampant in prisons.\u003cbr/\u003eBut doctors here have found methadone clinics aren't effective when limited to prisons.\u003cbr/\u003eThey have to be set in local communities so that released prisoners can continue their treatment. \u003cbr/\u003eThe AIDS Research centre has established departments offering free counselling and tests for people whose sexual activity puts them at high risk of getting AIDS. \u003cbr/\u003eBut sex outside of marriage is illegal in Iran and Professor Mohraz, who has spearheaded Iran's battle against AIDS for twenty years, says a further shift in attitude is vital. \u003cbr/\u003eOfficials are still conducting surveys to find out how many people are infected with HIV.\u003cbr/\u003eThey believe facilities have helped make a dent in the virus's transmission, but it's still growing and in 2008 the health ministry estimated there were between seventy to a hundred thousand cases.\u003cbr/\u003eProfessor Mohraz regrets the trend toward unsafe sexual behaviour.\u003cbr/\u003eShe and other AIDS specialists are calling for programmes to be extended to other vulnerable groups.  \u003cbr/\u003eThere is protection for homosexuals, prostitutes and people who commit adultery.\u003cbr/\u003eThey are all illegal and while condoms are distributed in prison, they're restricted to conjugal visits. \u003cbr/\u003eMohraz says \"unfortunately our epidemiology pattern has shifted toward (unsafe) sexual intercourse. Regarding the sexual relations, public awareness is very effective but unfortunately mass media have remained silent about it. They don't talk about sexual intercourse and using condoms.\"\u003cbr/\u003eTo counter the public silence, this research centre has an education department which trains people from throughout the Middle East.\u003cbr/\u003eThe (HIV) \"Positive Club\" next door offers a range of facilities allowing people to share their experiences, enjoy language classes and take part in music workshops.\u003cbr/\u003eThis man is a former patient and is now the Club's arts director.\u003cbr/\u003eHe says \"in the past four and half years I have trained 30 patients to master in Moaragh Kari (inlaid wood work) which is a very difficult task. experts say the time has come to widen the frontier of the battle against AIDS by raising public awareness and making condoms more
