Wide of midwife and students in class\u003cbr/\u003eIf you''re pregnant Sierra Leone is one of the riskiest places in the world to be.\u003cbr/\u003eMore women and their babies die in childbirth in the west African state than anywhere else on earth according to UNICEF. \u003cbr/\u003eNow to commemorate its 49th Independence Day, the country is launching a raft of free services to save the lives of its women and children.\u003cbr/\u003eA woman lies on an operating table fearful of the caesarean surgery she needs to keep her baby alive.\u003cbr/\u003eSadly, like too many women in Sierra Leone, she didn''t survive.\u003cbr/\u003eThe risk of a woman here dying from complications during pregnancy and childbirth is one in eight according to UNICEF. \u003cbr/\u003eIn 2008, the under fives mortality rate was 140 per 1000 live births and maternal mortality ratio 857 per 100,000 live births. \u003cbr/\u003eOne in twelve children still die before their first birthday and the majority of these deaths are caused by preventable diseases.\u003cbr/\u003eAccess to medical services for pregnant women and children in Sierra Leone is very limited. \u003cbr/\u003eWith a population of about 5.5 million people, the country has only one maternity hospital facility to cater for about one million children.  \u003cbr/\u003ePrinces Christian Maternal Hospital PCMH caters for about 250 thousand pregnant women and new mothers.\u003cbr/\u003eAccording Chief Medical Director Dr. Ibrahim Thorlie each doctor sees between 30 to 50 patients a day.\u003cbr/\u003e\"For every hundred thousand women (which give birth), up to seven to eight hundred of them will die,\" he says. \u003cbr/\u003eDr Thorlie says this is largely down to poverty.\u003cbr/\u003e\"When we check the causes of maternal and child deaths, we found out that because the patients have to pay a fee for consultation and other things, that is why they refuse to come to the hospital. They come to the hospital when they are about to die and that is why a lot of them die,\" he says. \u003cbr/\u003eToday (April 27 2010), on Sierra Leone''s Independence Day the government is to declare that health care for pregnant and breastfeeding women and all children under five will be free.\u003cbr/\u003eThe initiative will deliver an essential package of healthcare services including emergency obstetric care and maternal and child health preventive services such as family planning, immunisation and insecticide treated bednets. \u003cbr/\u003eThese services will be delivered free of charge at all public health facilities in all 13 districts across the country. \u003cbr/\u003eIt''s estimated that 230,000 pregnant women and nearly one million infants will benefit from the free healthcare services in 2010 alone. \u003cbr/\u003eSome doubt the President Koroma and his All People''s Congress (APC) government will live up to their promise, but the Ministry of Health and Sanitation has already recruited 21 Sierra Leonean trained medical doctors and 35 midwives.  \u003cbr/\u003eCuban and Nigerian medical doctors and midwives have be sent into government hospitals in the provinces and Koroma has also promised to give out performance based incentives for medical workers, an increment on doctor''s salaries and better working conditions for medical staff.\u003cbr/\u003e\"We are fully prepared to host the free care services from April, 27th in terms of personnel, equipment, and infrastructure,\" says Dr. Ibrahim Bundu, a senior surgeon at the Makeni hospital - the government''s new school for midwives in Maken that is offering training to maternal and child health assistants.\u003cbr/\u003e\"We have so many facilities in this school and because of these facilities we are sure of giving quality training to our students,\" says Sister Frances Fornah, Head of the School of Midwifery. \u003cbr/\u003eIt''s welcome news and both staff and mums to be are hopeful that the reforms will create real change.\u003cbr/\u003e\"Since we are now going to have free medical care, the death rate on us will reduce because we would have encouragement at the hospital and get proper care,\" says new mum,  Muskuda Massaquoi. \u003cbr/\u003eBut the government is still preparing a long term plan.\u003cbr/\u003eAs it stands it has $71 million Dollars for the project - that''s $20 million short of the money needed  to fully implement the first phase of the free health care scheme.\u003cbr/\u003eThe Ministry of Health and Sanitation is collaborating with UNICEF, UK Aid, the World Bank, the WHO, UNFPA, Save the Children, Search for Common Grounds and civil society organisations to ensure the successful implementation of the
