But now some doctors are trying out a new device which targets sweat glands.
Summer in the city and the office workers are among those seeking the relief of shade and a breeze.
For most of us sweating is normal, perspiration is the body's way of regulating our temperature.
But some people suffer from excessive sweating and doctors call this hyperhidrosis.
It can affect the armpits, the back and other areas such as hands and feet.
Doctors who treat patients with the problem say it may not be incapacitating, or painful, but it can cause people severe embarrassment and shame, this can affect them psychologically and have an impact on them at work.
Not all sweating is a response to heat, it can be an emotional response such as stress.
Musicians who suffer with excessively sweaty hands are among those who seek treatment.
This is the Courthouse Clinic in Essex which already offers treatments for patients with hyperhidrosis.
Treatment can range from deodorants to drugs and botox.
But today the staff are trying out something new, radiofrequency.
The machine is called SweatX.
It transmits high radiofrequency energy into the tissue.
This causes heat beneath the surface of the skin where the sweat glands sit.
It's manufacturer claims that because hyperactive sweat glands are saturated with water, compared to the surrounding tissue, the heat selectively targets the sweat glands.
Courthouse Clinic's resident physician Dr. Stephen Robertson usually treats his patients with botox, but he believes the new treatment offers an alternative for people who have sweating over an area too wide to be covered by botox.
He says: "With the SweatX you've got radio frequency which is, it could be thought of as laser because it's creating energy, but laser is actually light and this is electric radio waves basically and these radio waves work by being effectively sent into the skin and the closest analogy I can think of to that is when you listen to a mobile phone it gets hot and again that's in effect a radio wave."
The clinic here stops short of describing this as a permanent solution. It's relatively new and there isn't follow up data available to show its effective for more than a period of six months.
Also there are many sweat glands in any one area of our bodies, so it's not possible to target all of them even in one area with one session.
The company's own study conducted by First Moscow State Medical University tested the procedure on twenty patients including three men with a control group of six women and four men.
It concluded the procedure reduced perspiration to varying degrees the majority for up to six months.
This clinic offers this as a four treatment package costing from 575 ($969 US), which is less than botox, but it isn't cheap.
Robertson says: "You are actually heating up the gland to the point of thermolysis and destruction. So the question is that gland is now gone, does a new one regrow? Does the body produce more sweat glands as a regenerative process."
According to Robertson, the treatment is safe providing it is carried out by a trained therapist.
There is a risk of burning which is why the therapist constantly takes the temperature of the woman she is treating.
Robertson says after the treatment some people may experience more sweating in other areas of their bodies.
He says: "Eccrine glands which control the heat regulation and are the non odourless ones which secrete the perspiration for want of a better word, are all over the body so to eliminate them from one specific area, although you get a slight compensatory increase elsewhere, is going to balance out because the body's capacity to lose heat through all its other areas is going to be quite adequate."
Several companies are producing machines to tackle the problem of excessive sweating.
One produces a microwave heat and has been approved as effective by America's FDA.
Consultant dermatologist Dr. Martin Wade has not used the Sweatx machine, but he says it may be an exciting alternative treatment for hyperhidrosis.
But he points out that excessive sweating can be caused by certain medical conditions and responsible physicians will look for underlying reasons for the problem before using radio frequency, or other methods of perspiration control.
According to Wade: "It can be due to an underlying medical condition such as a hormonal imbalance, a nerve injury, drug intake, but the majority of cases that we see of hyperhidrosis are what is known as primary hyperhidrosis and this is where we don't find an underlying cause for the condition."
According to Wade: "The obvious attraction of this type of treatment is that it has a more permanent effect than something like botulinum toxin which does wear off and the radio frequency devices are generally easy to use and have a good patient acceptance with low problems with pain."
This treatment has only been cleared for use on the armpits, not the hands or feet. If you suffer the problem over a large area there are other treatments that doctors like Wade consider.
He says: "On a very simple level it (the treatment) might be using a very strong antiperspirant for auxiliary hyperhidrosis, these can also be used for the hands and the feet as well, but if it's more severe if it's sweating all over the body then other treatment may be needed such as systemic medication, or tablets. This can have an effect to block the cholinergic nerves in the body which are the ones that stimulate the sweat glands and whilst it can cause a reduction in sweating it can also have other side effects. These can include a dry mouth, blurred vision, constipation or urinary retention, so there's always a trade off with the side effect profile and getting the right dose for treating the underlying condition."
In the UK botox has been regularly used to treat patients with hyperhidrosis, but doctors like Robertson says injections in the hands and feet can be too painful for some people.
There is concern that the new types of treatment might cause contractions in the skin, but Wade says: If it's done under supervision and appropriately, then we don't cause enough damage to cause scarring, or contraction, so that why we're preferentially heating the sweat glands, but not all of the underlying tissue."
In Wade's opinion the the treatment would not be suitable for pregnant women.
London, UK, July 23, 2014
1.
